9224-4755 Québec inc., a legal person governed by the Business Corporations Act (Quebec) (hereinafter called “PetalMD.com”), is committed to protecting the privacy of your information, including personal information. This Privacy Statement describes PetalMD.com’s Web site privacy practices.
Under this Privacy Statement, PetalMD.com agrees to comply with the Personal Information Protection and Electronic Documents Act (Canada) and the Act respecting the protection of personal information in the private sector (Quebec) (together the “Law”).
PetalMD.com shall abide by the requirements of the Law governing any personal information, that is to say any information concerning a physical person that could be used to identify him or her.
PetalMD.com is responsible for the personal information which it manages, and it has appointed a personal information management coordinator who will ensure compliance with the Law.
1. Information Collected
PetalMD.com offers a variety of applications and services that are collectively referred to as the “Services”. PetalMD.com collects information from individuals who visit PetalMD.com’s Web site (“Visitors”) and individuals who register to use the Services (“Users”).
When you express an interest in obtaining additional information about the Services or registering to use the Services, PetalMD.com requires you to provide personal contact information, such as your name, the name of your medical group, your medical specialty, your phone number, and your email address (“Required Contact Information”). When you register for the Services, the PetalMD.com Web site requires you to provide PetalMD.com with your group name, your hospital affiliation, and the names, email addresses and license numbers of the group’s physicians. Required Contact Information and Optional Information (as identified in PetalMD.com’s Web site) are collectively referred to as “Data About PetalMD.com Users”.
As you navigate PetalMD.com’s Web site, PetalMD.com may also collect information through the use of commonly-used information-gathering tools, such as cookies (“Web Site Navigational Information”). Please refer to Section 3 below for more information about Web Site Navigational Information.
PetalMD.com collects personal information only for the purposes detailed above.
2. Use of Information Collected
PetalMD.com uses Data About PetalMD.com Users to perform the services requested. For example, if you fill out a “Register” Web form, PetalMD.com will use the information provided to contact you about your interest in the Services.
PetalMD.com may also use Data About PetalMD.com Users for marketing purposes. For example, PetalMD.com may use information you provide to contact you to further discuss your interest in the Services and to send you information regarding PetalMD.com, such as information about promotions or events.
PetalMD.com uses Web Site Navigational Information to operate and improve its Web sites. PetalMD.com may also use Web Site Navigational Information alone or in combination with Data About PetalMD.com Users to provide personalized information about PetalMD.com.
Each and every employee of PetalMD.com who collects, stores or uses personal information is required to ensure that the collection, use and disclosure of this information is done in compliance with this Privacy Statement and with the relevant procedures.
3. Web Site Navigational Information
PetalMD.com uses commonly-used information-gathering tools, such as cookies, to collect information as you navigate PetalMD.com’s Web sites (“Web Site Navigational Information”), including standard information from your Web browser (such as browser type and browser language), your Internet Protocol (“IP”) address, and the actions you take on PetalMD.com’s Web sites (such as the Web pages viewed and the links clicked).
This Section describes the types of Web Site Navigational Information that may be collected on PetalMD.com’s Web sites and how this information may be used.
Cookies
PetalMD.com uses cookies that are session-based and persistent-based. Session cookies exist only during one session. They disappear from your computer when you close your browser software or turn off your computer. Persistent cookies remain on your computer after you close your browser or turn off your computer.
Each time you log into the Services, a session cookie containing an encrypted, unique identifier that is tied to your account is placed in your browser. These session cookies allow PetalMD.com to uniquely identify you when you are logged into the Services and to process your online transactions and requests. Session cookies are required to use the Services.
PetalMD.com’s Web site uses persistent cookies that only PetalMD.com can read and use to identify browsers that have previously visited PetalMD.com’s Web sites. When you use the Services, a unique identifier is assigned to you. This unique identifier is associated with a persistent cookie that PetalMD.com places on your Web browser. PetalMD.com is especially careful about the security and confidentiality of the information stored in persistent cookies. For example, PetalMD.com does not store account numbers or passwords in persistent cookies. If you disable your Web browser’s ability to accept cookies, you will be able to navigate PetalMD.com’s Web sites, but you will not be able to successfully use the Services.
PetalMD.com may use information from session and persistent cookies in combination with Data About PetalMD.com Users to provide you with information about PetalMD.com and the Services.
IP Addresses
When you visit PetalMD.com’s Web site, PetalMD.com collects your Internet Protocol (“IP”) addresses to track and aggregate non-personal information. For example, PetalMD.com uses IP addresses to monitor the regions from which Users and Visitors navigate PetalMD.com’s Web sites.
PetalMD.com also collects IP addresses from Users when they log into the Services as part of PetalMD.com’s “Identity Confirmation” and “IP Range Restrictions” security features.
4. Refer a Colleague and User Testimonials
PetalMD.com may provide message boards, blogs, or chat rooms on PetalMD.com’s Web sites. Any personal information you choose to submit in such a forum may be read, collected, or used by others who visit these forums, and may be used to send you unsolicited messages. PetalMD.com is not responsible for the personal information you choose to submit in these forums.
Users and Visitors may elect to use PetalMD.com’s referral program to inform colleagues about PetalMD.com’s Web sites. When you use the referral program, PetalMD.com requests the colleague’s name and email address. PetalMD.com will automatically send the colleague a one-time email inviting him or her to visit PetalMD.com’s Web sites.
PetalMD.com posts a list of Users and testimonials on its Web sites that contain information such as User names and titles. PetalMD.com obtains the consent of each User prior to posting any information on such a list or posting testimonials.
5. Sharing of Information Collected
Unless described in this Privacy Statement, PetalMD.com does not share, sell, rent, or trade any personal information provided with third parties for their promotional purposes.
Personal information is not used or disclosed for purposes other than for which it was collected unless the person concerned consented thereto or if it is authorized under the Law. PetalMD.com shall store personal information only as long as it is necessary for the determined purposes.
Nevertheless, PetalMD.com reserves the right to use or disclose information provided if it reasonably believes that use or disclosure is necessary to protect PetalMD.com’s rights and/or to comply with a judicial proceeding, court order, or legal process.
6. Correcting and Updating Your Information
Users may update or change their registration information by editing their user record. To update a user profile, please login to http://www.petalmd.com with your PetalMD.com username and password and click “Modify my profile” To update an organization’s information, please login to http://www.petalmd.com with your PetalMD.com username and password and send a message to PetalMD Support.
Every User from whom PetalMD.com has collected personal information is entitled to modify his or her file to correct inaccurate, incomplete or ambiguous information and to have outdated information struck out, as well as information that is not warranted by the purpose of the file, or to make written comments and have them filed on record.
7. User Data
PetalMD.com Users may electronically submit data or information to the Services for hosting and processing purposes (“User Data”). PetalMD.com will not review, share, distribute, or reference any such User Data except as provided in the PetalMD.com Terms of Use, or as may be required by law. In accordance with the PetalMD.com Terms of Use, PetalMD.com may access User Data only for the purpose of providing the Services, preventing or addressing service or technical problems, at a User’s request in connection with User support matters, or as may be required by law.
PetalMD.com shall advise any person who requests it of the existence of personal information that concerns him or her, of the use made of such information, and whether it has been disclosed to third parties, and shall allow that person to have access to such information. It is also possible to contest the accuracy and completeness of such information and to make the appropriate corrections.
Please note that access to your file containing your personal information is free of charge. However, we may require a reasonable fee for the reproduction and forwarding of personal information. We will notify you ahead of time of the amount of these fees if applicable.
8. Security
PetalMD.com uses robust security measures to protect User Data from unauthorized access, maintain data accuracy, and help ensure the appropriate use of User Data. When the Services are accessed using Internet Explorer version 7.0 or later, Firefox version 2.0 or later, Safari version 3.0 or later, or Google Chrome, Secure Sockets Layer (SSL) technology protects User Data using both server authentication and data encryption. These technologies help ensure that User Data is safe, secure, and only available to the User to whom the information belongs and those to whom the User has granted access. PetalMD.com also implements an advanced security method based on dynamic data and encoded session identifications, and PetalMD.com hosts its Web sites in a secure server environment that uses firewalls, intrusion detection systems, and other advanced technology to prevent interference or access from outside intruders. Users are responsible for maintaining the security and confidentiality of their PetalMD.com usernames and passwords.
PetalMD.com stores your information within the territorial limits of Quebec.
9. Changes to this Privacy Statement
PetalMD.com reserves the right to change this Privacy Statement. PetalMD.com will provide notification of the material changes to this Privacy Statement through PetalMD.com’s Web sites at least thirty (30) business days prior to the change taking effect.
